 Important information... 

Kindergarten Enrollment 2010 - 2011

Enrollment packets are available in the office.  The following documents are required by Arkansas State Law and must be provided with the completed enrollment papers when the child is enrolled.  

 1)  Proof of your child's age (birth certificate, statement by the local registrar or a county recorder certifying the child’s date of birth, attested baptismal certificate, passport, affidavit of the date and place of birth by the child’s parent or guardian, previous school record or United States military identification) Age requirement is five (5) years old on/before August 15, 2010.

2)  Social Security Card or number OR a waiver number assigned by the school 

3)  Up-to-date Immunization Record            

        4 - DTaP/DTP/Td (diptheria, tetanus, acellular pertussis) Three doses before fourth birthday AND fourth dose after fourth birthday                        

        3 - IPV/OPV (Polio) Two doses before fourth birthday AND third dose after fourth birthday OR 4 doses total regardless of age

        2 - MMR (measles, mumps, rubella) First dose after first birthday AND second dose at least 28 days after first dose

        3 - Hepatitis B before kindergarten enrollment

          2 - Varicella (chickenpox) First dose after first birthday AND second dose at least 28 days after first dose

Keep saving those Box Tops for Education over the summer.  Thanks to you, over $900.00 was raised for the library's Accelerated Reading Program during the 2009-2010 school year.
